ABSTRACT

Objective@#To explore the relationship between handgrip strength (HGS) and pulmonary function (PF) in college students with sedentary lifestyle, so as to provide a reference for taking HGS as an essential factor of PF.@*Methods@#In March 1-5, 2023, a total of 44 college students were recruited and were divided into the sedentary group (22) and exercise group (22) according to the International Physical Activity Questionaire (IPAQ), with 22 students in each group. Independent sample t-test was used to compare the indexes between groups, pearson correlation coefficient was used to determine the correlation between HGS and PF. Multiple linear regression analysis was used to determine the predictive model of PF.@*Results@#There were statistical significance of the correlations between HGS and FVC, MEP, PEF, FEV1, FIVC, MIP, and PIF ( r=0.79, 0.47, 0.44, 0.60, 0.72, 0.53 , 0.49, P <0.01). When gender, physical activity, age, height, weight, and HGS were included as predictors in the regression model, height and HGS had significant effects on FVC ( R 2=0.75, F= 60.55 , P <0.01), weight and HGS had a good predictive effect on FIVC ( R 2=0.67, F=41.77, P <0.01).@*Conclusion@#HGS is significantly associated with PF in college students with sedentary lifestyle. Therefore, HGS can be used as an important indicator to predict the PF status of habitual sedentary college students.

ABSTRACT

OBJECTIVE To study the extraction and enrichment technology of chrysosplenides A (CA) and I (CI) in Tibetan medicine Chrysosplenium axillare. METHODS HPLC method was used to determine the contents of CA and CI. The orthogonal experiment was used to optimize the extraction technology of CA and CI in C. axillare using total transfer rate of CA and CI as evaluation indexes, with volume fraction of ethanol, extraction temperature, extraction times and solid-liquid ratio as factors. The validation test was also performed. The enrichment technology of CA and CI in C. axillare was optimized using D101 macroporous adsorption resin as adsorbent, total contents of CA and CI as evaluation indexes, with the volume fraction and dosage of eluent for impurities and target components. The validation test was also performed. RESULTS The optimum extraction conditions of CA and CI from C. axillare were as follows: the medicinal powder of C. axillare was extracted by ultrasound at room temperature for 45 min at one time with 8 times of 50% ethanol. Results of validation tests showed that total transfer rate of CA and CI in C. axillare was 95.43% in average (RSD=1.02%, n=3). The optimal enrichment technology was as follows: the sample solution was added into D101 macroporous adsorption resin column and stood for 1 hour; the impurities were eluted with 20% ethanol 4 BV (column volume), and CA and CI were eluted with 50% ethanol 4 BV. The results of validation tests showed that total content of CA and CI was 322.7 mg/g in average (RSD=1.05%, n=3), with average enrichment multiple of 11.61 times. CONCLUSIONS The study has successfully optimized the extraction and enrichment technology of CA and CI from C. axillare, and can provide reference for the development and utilization of CA and CI.

ABSTRACT

Two prenylated 2-arylbenzofurans were isolated from roots of Artocarpus heterophyllus, with a combination of various chromatographic approaches, including ODS, MCI, Sephadex LH-20, and semipreparative high performance liquid chromatography(HPLC). They were identified as 5-[6-hydroxy-4-methoxy-5,7-bis(3-methylbut-2-enyl)benzofuran-2-yl]-1,3-benzenediol(1) and 5-[2H,9H-2,2,9,9-tetramethyl-furo[2,3-f]pyrano[2,3-h][1]benzopyran-6-yl]-1,3-benzenediol(2) with spectroscopic methods, such as HR-ESI-MS, IR, 1D NMR, and 2D NMR, and named artoheterins B(1) and C(2), respectively. The anti-respiratory burst activities of the two compounds were evaluated with rat polymorphonuclear neutrophils(PMNs) stimulated by phorbol 12-myristate 13-acetate(PMA). The results showed that 1 and 2 exhibited significant inhibitory effect on respiratory burst of PMNs with IC_(50) values of 0.27 and 1.53 μmol·L~(-1), respectively.

ABSTRACT

Obvious epigenetic differentiation occurred on Lycium barbarum in different cultivation areas in China. To investigate the difference and change rule of DNA methylation level and pattern of L. barbarum from different cultivation areas in China, the present study employed fluorescence-assisted methylation-sensitive amplified polymorphism(MSAP) to analyze the methylation level and polymorphism of 53 genomic DNA samples from Yinchuan Plain in Ningxia, Bayannur city in Inner Mongolia, Jingyuan county and Yumen city in Gansu, Delingha city in Qinghai, and Jinghe county in Xinjiang. The MSAP technical system suitable for the methylation analysis of L. barbarum genomic DNA was established and ten pairs of selective primers were selected. Among amplified 5'-CCGG-3' methylated sites, there were 35.85% full-methylated sites and 39.88% hemi-methylated sites, showing a high degree of epigenetic differentiation. Stoichiometric analysis showed that the ecological environment was the main factor affecting the epigenetic characteristics of L. barbarum, followed by cultivated varieties. Precipitation, air temperature, and soil pH were the main ecological factors affecting DNA methylation in different areas. This study provided a theoretical basis for the analysis of the epigenetic mechanism of L. barbarum to adapt to the diffe-rent ecological environments and research ideas for the introduction, cultivation, and germplasm traceability of L. barbarum.

ABSTRACT

Radiotherapy plays an important role in the treatment of all stages of pancreatic cancer, and some therapeutic effects have been obtained. At present, it needs to be considered and practiced in many aspects that how to play the role of radiotherapy in the treatment of pancreatic cancer. It is related to the choice of dose mode, the accuracy of irradiation, the cooperation of other treatment methods and other factors. The emergence of precision radiotherapy technology and the subsequent change in the idea of radiotherapy will further improve the role of radiotherapy in the treatment of pancreatic cancer. Based on the relevant literatures at home and abroad and our own experience, this paper reviews the role of precision radiotherapy for pancreatic cancer.

ABSTRACT

With the rapid development of radiotherapy equipment, imaging technology, and artificial intelligence, radiotherapy has entered the era of precision therapy. Radiotherapy is one of the important methods for the treatment of pancreatic cancer, and rapid progress has been made in related concepts and technical models of radiotherapy in recent years. With reference to the latest advances in radiotherapy technology and the change of radiotherapy dose mode, which the experts in the field of radiotherapy are concerned with, this article reviews and analyzes recent articles and points out that multimodal images play an important role in guiding radiotherapy for pancreatic cancer, and the dose mode of radiotherapy develops to high-dose hypofractionated radiotherapy. The effect of neoadjuvant radiotherapy has been further confirmed, and strong chemotherapy with the combination of multiple drugs is the main radiotherapy regimen for pancreatic cancer.

ABSTRACT

On the basis of literatures and standards relating to Tibetan medicine, the varieties, origin, standards and efficacy of Saxifragaceae plant used in Tibetan medicine were summarized. According to the findings, 75 species(including varieties) in 8 genera of Saxifragaceae plants, involving 21 varieties, are used in Tibetan medicine. Among them, 9 commonly used varieties, namely Songdi, Sedi, Yajima, Aoledansaierbao, Jiansidawu, Saiguo, Katuer, Sangdi, Maoqinghong, are recorded in Chinese Pharmacopoeia, Ministry Standards for Tibetan Medicine, Tibetan Medicine Standards and other local standards, accounting for 42.9% of the total number of varieties. Tibetan names, Tibetan translation of Chinese names, as well as original plant of Tibetan medicine varieties are quite different in relevant Tibetan medicine standards and literatures, which resulted in common phenomena of synonym and homonym. The standards of most varieties only involve characters, and microscopic, physical and chemical identification, with low quality standards. Based on the results of the analysis, this paper suggests strengthening surveys on herbal textual research, resources and current utilization of Saxifragaceae plants used in Tibetan medicine, summarizing the varieties, establishing improved quality standards, and perfor-ming a comparative study on therapeutic material basis and biological activity of different original plants, in order to promote rational use of these medicinal plant resources, and ensuring the accuracy, safety, and effectiveness of clinical medication.

ABSTRACT

OBJECTIVE@#To explore the mechanism of moxibustion on the treatment of rheumatoid arthritis (RA) in the perspective of programmed cell death-1 and its ligand 1 (PD-1/PD-L1).@*METHODS@#A total of 30 Japanese big ear white rabbits were randomly divided into a control group, a model group and a moxibustion group, 10 rabbits in each one. In the model group and the moxibustion group, RA model was prepared by the injection of Freund's complete adjuvant (FCA) into the hind knee joint cavities of each rabbit. In the control group, 0.9% sodium chloride solution of the same dose was injected. On the 8th day of experiment, in the moxibustion group, moxibustion was applied to "Shenshu" (BL 23) and "Zusanli" (ST 36), 5 cones at each acupoint, on the bilateral sides alternatively, once a day, 6 treatments as one course, with an interval of 1 days between the treatment courses. Totally, 3 courses of treatment were required. On the 1st, 7th, 14th, 21st and 28th days of experiment, successively, the circumference of the bilateral knee joints was measured with the tape. On the 28th day of experiment, H.E. staining was adopted to observe the histopathological morphology and to evaluate the score of knee synovial tissue. ELISA was used to determined the concentrations of soluble PD-1 (sPD-1) and its ligand 1 (sPD-L1), the interleukin 2 (IL-2) and IL-17 in knee synovial fluid and the concentrations of sPD-1 and sPD-L1 in serum. The histochemistry method was used to determine the expressions of membrane PD-1 (mPD-1) and its ligand 1 (mPD-L1) in spleen tissue.@*RESULTS@#On the 14th, 21st and 28th days of experiment, the circumference of both knee joints was increased in each of the rabbits in the model group as compared with the control group (<0.01), and it was reduced significantly in the moxibustion group as compared with the model group (<0.01). Compared with the control group, the hyperplasia of synovial tissue and fibrous tissue, as well as inflammatory cell infiltration were increased obviously in the model group (<0.01), and they were reduced significantly in the moxibustion group as compared with the model group (<0.01, <0.05). Compared with the control group, the concentrations of IL-2 and IL-17 in knee synovial fluid were increased in the rabbits of the model group (<0.01). Compared with the model group, after the intervention with moxibustion, the concentrations of IL-2 and IL-17 in knee synovial fluid were reduced in the rabbits of the moxibustion group (<0.05). Compared with the control group, the concentrations of sPD-1 and sPD-L1 in knee synovial fluid and serum in the rabbits of the model group were all increased (<0.05). Compared with the model group, the concentration of sPD-1 in the knee synovial fluid and serum were reduced in the rabbits of moxibustion group (<0.05). Compared with the control group, the expressions of mPD-1 and mPD-L1 in spleen tissue were increased obviously in the rabbits of the model group (<0.05). Compared with the model group, the expression of mPD-L1 in spleen tissue was up-regulated in the rabbits of the moxibustion group (<0.05).@*CONCLUSION@#Moxibustion could inhibit the over-activation of T cells by enhancing the negative regulation of PD-1/PD-L1 signaling pathway so as to play its effect in treatment of RA.

OBJECTIVE@#To investigate the effect of other gene mutations outside the fusion gene on the first complete remission (CR) induced by one course of induction chemotherapy in patients with core binding factor-associated acute myeloid leukemia (CBF-AML).@*METHODS@#DNA was extracted from bone marrow or peripheral blood samples of newly diagnosed CBF-AML patients admitted to the Hematology Department of the Second Hospital of Shanxi Medical University from January 2015 to January 2019. Next-generation sequencing was used for detection of 34 kinds of hematologic malignancy-related gene mutations in patients with CBF-AML, the effect of related gene mutations on the first complete remission (CR) rate in one course of induction chemotherapy was analyzed by combineation with clinical characteristics.@*RESULTS@#34 kinds of genes in bone marrow or peripheral blood of 43 patients were detected by high throughput sequencing and the gene mutations were detected in 16 out of 34 genes. The mutation rate of KIT gene was the highest (48.8%), followed by NRAS (16.3%), ASXL1 (16.3%), TET2 (11.6%), CSF3R (9.3%), FLT3 (9.3%), KRAS (7.0%). The detection rates of mutations in different functional genes were as follows: genes related with signal transduction pathway (KIT, FLT3, CSF3R, KRAS, NRAS, JAK2, CALR, SH2B3, CBL) had the highest mutation frequency (72.1% (31/43); epigenetic modification gene mutation frequency was 30.2% (13/43), including ASXL1, TET2, BCOR); transcriptional regulation gene mutation frequency was 7.0% (3/43), including ETV6, RUNX1, GATA2). Splicing factor related gene mutation frequency was 2.3% (1/43), including ZRSR2). The CR rate was 74.4% after one course of induction chemotherapy. At first diagnosis, patients with low expression of WT1 (the median value of WT1 was 788.9) were more likely to get CR (P=0.032) and the RFS of patients who got CR after one course of induction chemotherapy was significantly longer than that of patients without CR [7.6 (2.2-44.1) versus 5.8 (1-19.4), (P=0.048)]. The rate of CR in the signal transduction pathway gene mutation group was significantly lower than that in non-mutation group (64.5% vs 100%) (P=0.045), while the level of serum hydroxybutyrate dehydrogenase (HBDH) was significantly higher than that in non-mutation group [(418 (154-2702) vs 246 (110-1068)] (P=0.032). There was no difference in CD56 expression between the two groups (P=0.053), which was limited to the difference between (≥20%) expression and non-expression. (P=0.048).@*CONCLUSION@#CBF-AML patients with signal transduction pathway gene mutation are often accompanied by high HBDH level and CD56 expression, moreover, the remission rate induced by one course of treatment is low.

ABSTRACT

Objective: To investigate the chemical constituents of leaves of Dendrobiumofficinale. Methods: Compounds were isolated from the leaves of D.officinale by column chromatography over Sephadex LH-20, MCI GEL CHP-20P, and ODS as well as by preparative HPLC. Their structures were identified by the analysis of their physical and chemical properties and the spectra data of NMR and MS. Results: Twenty-four compounds were isolated from the leaves of the plant, namely 3,4-dihydroxy-5,4'-dimethoxy bibenzyl (1), moscatilin (2), 4,4'-dihydroxy-3,5-dimethoxybibenzyl (3), densiflorol A (4), (S)-3,4,α-trihydroxy-5,4'-dimethoxybibenzyl (5), gigantol (6), dendrocandin U (7), dendrocandin B (8), loliolide (9), (6R,9S)-dihydroxy-megastigma-4,7-dien-3-one-9-O-β-D-glucopyranoside (10), (6R,9S)-9-hydroxy-megastigma-4,7-dien-3-one-9-O-β-D-glucopyranoside (11), (+)-syringaresinol (12), rutin (13), 2-benzothiazolol (14), p-hydroxyacetophenone (15), p-hydroxyl-benzoic acid (16), protocatechuic acid (17), catechol (18), ethyl p-hydroxyhydrocinnamate (19), 1-glycerol linolenate (20), 2-butoxyethyl linolenate (21), palmitic acid (22), octadecadienoic acid-2,3-dihydroxypropyl ester (23), and urticifolene (24). Conclusion: Itis the first report of the occurrence of compounds 10, 11, 14-23 in Orchidaceae family. Compounds 1, 2, 4, and 6are found in D. officinale for the first time.

ABSTRACT

OBJECTIVE@#To investigate the influence of influence of combination with 1q21 amplification or-no in patients with newly diagnosed MM on the clinical effecacy of bortezomib-based induction chemotherapy and long-term prognosis of patients.@*METHODS@#148 patients with newly diagnosed MM treated from January 2010 to May 2018 were selected and divided into 2 groups: group A (70 patients) without 1q21 amplification and group B (78 patients) with 1q21 amplification; and the survival benefit and influence on clinical efficacy of bortezomib were compared between 2 groups, and the factors influencing clinical prognosis in the patients with newly diagnosed MM were analyzed.@*RESULTS@#The median PFS and OS of patients in B group were significantly shorter than those in group A (P<0.05). There was no significant difference in the median OS and PFS between patients with 1q21 amplification copies number =3 and >3 (P>0.05). Multivariate Cox model analysis indicated that the adverse factors for OS were ISS staging, Hb levels, β2 microglobulin levels and 1q21 amplification respectively, and the adverse factors for PFS were Hb levels and 1q21 amplification respectively in patients with newly diagnosed MM (P<0.05). The very good partial remission rate of newly diagnosed MM patients with 1q21 amplification and bortezomib-based induction chemotherapy were significantly higher than that in the patients without bortezomib-based induction chemotherapy (P<0.05). The median PFS time of newly diagnosed MM patients with 1q21 amplification and auto-HSCT after bortezomib-based induction chemotherapy was significantly longer than that of patients without auto-HSCT (P<0.05).@*CONCLUSION@#1q21 amplification should be the independent risk factor for poor prognosis of patients with newly treated MM. The application of bortezomib-containing induction chemotherapy in patients with 1q21 amplification can efficiently improve the remission rate, while auto-HSCT consolidation therapy may prolong patients' PFS.

ABSTRACT

Objective:To investigate the related risk factors for biliary anastomotic stenosis after liver transplantation (LT) from donation after cardiac death(DCD) and therapeutic strategies.Methods:The data of 192 patients who received LT from DCD in First Hospital of Kunming from Jan 2010 to Jun 2018 were retrospectively analyzed. A total of 145 patients were enrolled, 85 males and 60 females, with average age 45 years. There was a biliary anastomotic stenosis in 8 cases and no stenosis in 137 cases. Their Chinese criterion for biliary anatomic stenosis, age, body mass index, liver fat, cold/warm ischemia time, unschedule cardiac arrest time, usage of vasopressors, high sodium in the donor were compared, and stenosis related factors were analysed by Spearman correlation analysis.Results:The stenosis was positively correlated with age ( r=0.229), body mass index ( r=0.204), lipoidosis ( r=0.239), duration of hot ischemia ( r=0.214), total duration of unplanned cardiac arrest ( r=0.401), use of booster drugs ( r=0.237), and preoperative donor hypernatremia ( r=0.557) (all P<0.05). Endoscopic biliary stent implantation is effective in the treatment of biliary anastomotic stenosis and has a high success rate. Conclusions:There are many factors related to biliary anastomotic stenosis after DCD liver transplantation, but the better donor maintenance, shorten cold/ warm ischemia time, improved anastomosis will be helpful to reduce biliary complications.As the same time, endoscopic biliary stent placement is the preferred way to treat biliary anastomotic stenosis.

ABSTRACT

Chrysosplenium nudicaule,Tibetan name " Yajima",is recorded as an effective medicine for the treatment of liver and gallbladder diseases by Tibetan Pharmacopoeia published in the past dynasties,but its traditional efficacy has not yet been investigated by means of modern pharmacological research methods. In this paper,the protective effect of extract of C. nudicaule(ECN) on liver injury in mice was observed by using the mice model of intrahepatic cholestasis(IC) induced by α-naphthyl isothiocyanate(ANIT) and the possible mechanism by which ECN work as the therapeutic agent was discussed. The results showed that the serum levels of AST,ALT,ALP,DBIL,TBIL and TBA of the model mice were notably reduced in dose-dependent manner(P<0. 01,P<0. 05). The activity of SOD and GSH-Px in the liver homogenate of mice was increased,while the content of MDA was decreased(P<0. 01,P<0. 05).Pathological examination of liver in mice showed that ECN could improve the pathological changes of liver tissue in mice. The mRNA expression level of genes related to bile acid metabolism were detected by RT-PCR and the results suggested that ECN could significantly increase the expression of genes such as BSEP,FXR and MRP2(P<0. 01,P<0. 05),meanwhile significantly reduce the expression of CYP7 A1(P<0. 01,P<0. 05). These results confirmed the protective effect of ECN on intrahepatic cholestasis-induced liver injury in mice,and indicated that the mechanism may be related to activating FXR and its target genes,reducing bile acid synthesis and increasing bile acid excretion. This study provides a modern pharmacological basis for the clinical application of Yajima in Tibetan medicine.

ABSTRACT

Objective To investigate the effect of low level laser treatment combined with periodontal therapy on interleukin-8 (IL-8), basic fibrobast growth factor (b-FGF) and interleukin-1β (IL-1β) level in gingival crevicular fluid on peri-implantitis , so as to evaluate the curative effect . Methods A total of 38 patients (50 implants) with early peri-implantitis were randomly divided into experimental group and control group, with 19 patients (25 implants) in each group. The patients in the experimental group were underwent level laser treatment combined with periodontal initial, and in the control group were only treated with periodontal initial. The implant gingival crevicular fluid was collected before the treatment (baseline), and 4 weeks and 12 weeks after the treatment. The levels of IL-8, b-FGF and IL-1βwere determined by enzyme-linked immunosorbent assay (ELISA). The SPSS19.0 software was used for the statistically analysis. Results The levels of IL-8, b-FGF and IL-1β of the experimental group and the control group were both improved at 4 weeks and 12 weeks after treatment comparing with that at the baseline, and the differences were statistically significant (all P<0.01). The differences in IL-8 and b-FGF levels in the gingival crevicular fluid were statistically significant at different time points for each group (all P<0.01). For the two groups, the differences in IL-1βwere not statistically significant at 4 weeks and 12 weeks after the treatment (all P>0.05). The levels of IL-8 and IL-1βof the experimental group were lower than those of the control group at 4 weeks after treatment, and the differences were statistically significant (all P<0.05). The level of b-FGF of the experimental group was higher than that of the control group at 12 weeks after the treatment, and the difference was statistically significant (P<0.05). Conclusions Low-energy laser can affect the level of cytokines in the gingival crevicular fluid on peri-implantitis. The combination of low level laser treatment and periodontal initial therapy is effective for treating peri-implantitis.

ABSTRACT

OBJECTIVE@#To verify the effect of warm acupuncture combined with western medicine in patients with acute exacerbation chronic obstructive pulmonary disease (AECOPD) with phlegm-turbid obstructing of the lung.@*METHODS@#Ninety patients with AECOPD were randomly divided into a western medicine group, a warm acupuncture group and a sham acupuncture group, 30 cases in each group. Routine treatment according to the Global initiative for chronic obstructive lung disease (GOLD) guidelines was used in the western medicine group. On the basis of the routine treatment, the warm acupuncture was applied at Fenglong (ST 40), Feishu (BL 13), Zusanli (ST 36) and other acupoints in the warm acupuncture group. In the sham acupuncture group, non-acupoints were taken on the basis of conventional treatment, and superficial acupuncture was performed for 1-3 mm, once a day for 2 weeks. Forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC), FEV1/FVC, cough and sputum assessment questionnaire (CASA), COPD assessment test (CAT), and the change of TCM syndrome score were observed before and after treatment in all groups.@*RESULTS@#After treatment, the FEV1, FVC and FEV1/FVC in each group were higher than those before treatment (0.05). The CASA score, CAT score and TCM syndrome score in each group were lower than those before treatment (0.05).@*CONCLUSION@#Warm acupuncture has a significant effect on the improvement of symptoms in patients with AECOPD with phlegm-turbid obstructing of the lung.

ABSTRACT

Objective To investigate the levels of T helper cell 17 (Th17), Th17-related cytokines in-terleukin 17 (IL-17) and interleukin 23 (IL-23) and regulatory T cell (Treg) in relapsing remitting multiple sclerosis (RRMS). Methods In a case-control study, plasma was collected from RRMS patients (n=20) and healthy subjects as control group (n=20). The percentages of Th17 and Treg cells and the levels of IL-17 and IL-23 were tested. The levels of Th17, Treg, IL-17 and IL-23 of the two groups were compared. Patients were treated with methylprednisolone. The levels of Th17, Treg, IL-17 and IL-23 of multiple sclerosis (MS) patients b efore and after treatment were compared. Expanded disability status scale (EDSS) score and the number of Gd-enhancing lesions were evaluated in the case group. Statistical analysis was made by body mass index (IBM) statistical program for social sciences (SPSS) 17.0 software. Independent sample t test was conducted to compare the measurement data of the case group and the healthy control group, and enumeration data were compared by χ2 test; paired sample t test was performed to compare the data of the case group before and after treatment; Pearson correlation analysis was made forthe variables of the MS group before treatment. Results In the RRMS group, the percentage of Th17 cells in peripheral blood was significantly higher than the control group [(2.10±0.45)%vs (1.09±0.20)%](t=9.130, P<0.01), the levels of Th17-related cytokines IL-17 and IL-23 were remarkably higher than the control group (IL-17:t=19.843, P<0.01;IL-23:t=22.747, P<0.01), and the percentage of Treg cells was significantly lower than the control group [(1.33 ±0.30)%vs (2.52±0.30)%], (t=12.422, P<0.01). The levels of Th17 and IL-17 were positively associated with EDSS score (Th17: r=0.458, P<0.05; IL-17: r=0.480, P<0.05), there was no significant-correlation between the level of IL-23 and EDSS score (r=0.368, P>0.05), and Th17, IL-17 and IL-23 were positively correlated with the number of Gd-enhancing lesions (Th17: r=0.446, P<0.05; IL-17: r=0.544, P<0.05; IL-23: r=0.461, P<0.05). The levels of Th17, IL-17 and IL-23 in the RRMS group after the treatment with methylprednisolone were obviously decreased than before treatment (Th17: t=5.747, P<0.01; IL-17: t=9.967, P<0.01; IL-23: t=14.697, P<0.01), while that of Treg was apparently increased (t=10.050, P<0.01). Compared with the control group, the levels of Th17, IL-17 and IL-23 in the RRMS group after treatment were higher (Th17: t=6.889, P<0.01;IL-17:t=7.185, P<0.01;IL-23:t=13.284, P<0.01), and the percentage of Treg was lower (t=7.622, P<0.01). EDSS score of the RRMS group after treatment was remarkably decreased than before treatment(t=6.190, P<0.01), but the number of Gd-enhanced lesions after treatment was no significantiy changed (t=1.453, P>0.05). Conclusion Th17/Treg expression imbalance and Th17-related cytokines IL-17, IL-23 may participate in the pathological process of MS, and they might be therapeutic target for MS.

ABSTRACT

Objective To evaluate the effect of high-dose hypofractionated radiotherapy upon the quality of life (QOL) of patients diagnosed with pancreatic cancer. Methods In this prospective study, 50 patients with pancreatic cancer admitted to our hospital between 2016 and 2017 were recruited. All patients underwent high-dose hypofractionated helical tomotherapy. The prescription doses for PTV, CTV, and GTV were 50, 60, and 70 Gy in 15-20 fractions, 5 times per week. The QOL was evaluated by the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (QLQ-C30) and pancreatic cancer-specific (European Organization for Research and Treatment of Cancer QLQ-PAN26) questionnaires before, after, 1-month and 3-month after the radiotherapy to statistically compare the QOL changes before and after radiotherapy and subsequent follow-up. Results A total of 50 patients with pancreatic cancer were included. At the end of radiotherapy, the overall QLQ-C30 score did not differ from that before radiotherapy (P=0. 330). At 1 month after radiotherapy, the overall QLQ-C30 score was significantly higher than that before radiotherapy (P=0. 000). The overall QLQ-C30 scores did not significantly differ at 1-and 3-month after radiotherapy ( P=0. 665). At 3 months after radiotherapy, social function score was considerably decreased (P=0. 047), and the remaining function scores were equal to those at 1 month post-radiotherapy. The symptoms of fatigue in the QLQ-C30 questionnaire were slightly improved at the end of radiotherapy, which were mitigated at 1-month after radiotherapy and became stable at 3-month following radiotherapy. Pain, insomnia, loss of appetite and diarrhea did not significantly change at the end of radiotherapy, whereas were improved at 1 month after radiotherapy. Nausea and vomiting were aggravated at the end of radiotherapy ( both P=0. 000), restored to the level before radiotherapy at 1 month after radiotherapy, and the symptoms were similar at 1-and 3-month after radiotherapy. Financial difficulty was worsened at the end of radiotherapy (P=0. 046), acceptable at 1 month after radiotherapy and worsened at 3 months following radiotherapy. In the QLQ-PAN26 questionnaire, the symptoms of pancreatic pain (backache, nocturnal pain) were mitigated at the end of radiotherapy (P=0. 009, P=0. 000), and further alleviated at 1 month after radiotherapy. No significant difference was noted at 1-and 3-month after radiotherapy. The body weight loss was slightly mitigated compared with that before radiotherapy ( P= 0. 000 ), and further improved at 1 month after radiotherapy ( P=0. 024). No significant difference was noted at 1-and 3-month after radiotherapy ( P=0. 226). Conclusion High-dose hypofractionated radiotherapy can significantly enhance quality of life of pancreatic cancer patients at 1- and 3- month following radiotherapy.This study further demonstrated the clinical value of high-dose hypofractionated radiotherapy in improving the quality of life on the basis of previous studies showing that high-dose hypofractionated radiotherapy can significantly improve the local control rate and survival rate of pancreatic cancer.

ABSTRACT

Objective To observe the dynamic changes of serum CA19-9 level before and after the treatment of high-dose and low-fractionated Tomotherapy combined with chemotherapy for pancreatic cancer,and explore the corresponding prognostic,predictive and therapeutic effects.Methods The clinical characteristics and the survival of 75 patients with pancreatic cancer treated in the Department of Radiotherapy of Air Force General Hospital were analyzed retrospectively from December 2012 to June 2017.The effects of baseline CA19-9 and CA19-9 level at 4-6 weeks after the radiotherapy on patient prognosis were analyzed by Kaplan-Meier method and Log-rank test retrospectively.Results The median survival time (MST) was 13.8 months (2.0-58.0 months).The baseline serum CA19-9 was 1109 U/ml(4.70-70000 U/ml).The MST in the < 1109 U/ml group and ≥ 1109 U/ml group were 14.0 months (95% CI:10.04-22.96 months) and 9.0 months(95% CI:7.75-13.25 months) respectively (x2 =6.31,P <0.05).The corresponding median progression-free survival (mPFS) was 7.0 months(95% CI:5.58-8.42 months) and 5.0 months(95 % CI:3.81-6.20 months) respectively (x2 =8.51,P < 0.05).The median CA19-9 level at 4-6 weeks after radiotherapy was 397.7 U/ml (11.02-43 250 U/ml).A total of 38 cases (38/49,77.6%) declined,and 11 cases (11/49,22.4%) increased.The MST of the declined and increased groups were 14.0 months(95% CI:7.24-20.77 months) and 8.0 months(95% CI:2.69-13.31 months) respectively (x2 =11.19,P <0.05).The corresponding mPFS were 5.5 months(95 % CI:4.05-6.95 months) and 4.0 months (95 % CI:1.98-6.02 months) respectively (x2 =9.12,P < 0.05).The ROC curve determined that the cut-off for the decline of serum CA19-9 was 60.9% at 4 to 6 weeks after radiotherapy.The MST of ≥ 60.9% group and < 60.9% group were 33.5 months (95% CI:0.24-66.76 months)and 9.0 months(95% CI:6.92-11.08 months)respectively (x2 =13.80,P<0.05).The corresponding mPFSwere 11.0 months(95% CI:3.44-18.56 months)and 5.0 months(95% CI:3.91-6.09 months)respectively (x2 =8.71,P<0.05).Conclusions Patients with the baseline serum CA19-9 < 1109 U/ml had a better outcome after hypofraction tomotherapy combined with chemotherapy.The decline of serum CA19-9 at 4-6 weeks after radiotherapy can serve as a prompt and effective predictor of the outcome and the progression of patients.The decline of serum CA19-9 ≥60.9% in patients with pancreatic cancer is associated with a low possibility of disease progression in half a year,whose overall survival is better.

ABSTRACT

Objective To study the effectiveness of basic therapy combined with low energy laser irradiation therapy to the patients with peri-implantitis, and to clarify the effect of laser irradiation in the treatment process. Methods A total of 38 patients (50 implants) who were diagnosed with early peri-implantitis from September 2017 to March 2018 in the Department of Stomatology, Tianjin First Central Hospital were randomly divided into the laser treatment group (n=25) and the conventional treatment group (n=25). The control group received basic treatment, and the laser treatment group received basic treatment combined with low energy laser irradiation (808 nm GaAlAs semiconductor continuous laser, average output power 80 mw, energy density 4 J/cm2, facula area 0.28 cm2, 15 seconds irradiation for each implant bag). The GIgingival index (GI), plaque index (PLI), sulcus bleeding index (SBI) and probe depth (PD) were examined before and 2 weeks and 8 weeks after the treatment. Results At 2 weeks and 8 weeks after the treatments, the indexes of the two groups were both improved compared with the baseline, and the differences were statistically significant (all P<0.05). Compared with the control group, the GI was significantly decreased (P=0.018) at 2 weeks after the treatment, and the GI, PD, and SBI were significantly decreased at 8 weeks after the treatment (P<0.001, P=0.018, and P<0.001, respectively). Conclusions At an energy density of 4 J/cm2, low energy laser irradiation at a wavelength of 808 nm can accelerate tissue healing of peri-implant inflammation after basic treatments.
